About This Service

What Is an ADHD Assessment at Flourishington?

An ADHD assessment at Flourishington is a structured, multi-method clinical evaluation designed to determine whether Attention-Deficit/Hyperactivity Disorder is present, identify its specific presentation, and develop a clear, personalized plan to help you or your child thrive. Conducted by David Fowope, PMHNP-BC, our assessments go far beyond a brief screening questionnaire.

We understand that ADHD looks different across ages, genders, and life contexts. A child struggling in the classroom, a teenager failing to meet academic potential, or an adult who has spent years wondering why they can't seem to focus the way others do — each deserves an evaluation that considers their full picture. Our assessment process captures that complexity with clinical precision and compassionate attention.

Following your evaluation, you will receive a clear diagnostic formulation, a written summary of findings, and a comprehensive treatment plan that may include medication recommendations, behavioral strategies, referrals, and official documentation for school or workplace accommodations.

Who This Serves

ADHD Assessment Is Right for You

ADHD presents differently across every age group. Our clinically structured assessment is designed to capture every presentation — from the hyperactive 8-year-old to the quietly struggling adult who was never diagnosed.

Children (Ages 8+)

For parents concerned about inattention, hyperactivity, impulsivity, or behavioral challenges affecting school performance and home life.

Adolescents

For teens struggling with academic underperformance, disorganization, emotional dysregulation, or social difficulties that may signal undiagnosed ADHD.

Adults

For adults who have always felt "different" — chronic disorganization, job instability, relationship struggles, or a lifetime of near-misses that may finally have an explanation.

Previously Undiagnosed

For anyone who suspects ADHD but has never been properly evaluated — including women and girls whose inattentive presentations are frequently overlooked.

Signs You May Benefit from an ADHD Assessment

Many people with ADHD go years without a correct diagnosis. If any of the following sound familiar, it may be time to seek a proper evaluation.

Request Your Assessment
  • Chronic difficulty focusing on tasks, conversations, or reading — even topics you enjoy
  • Persistent disorganization, missed deadlines, and forgetfulness that feels uncontrollable
  • Hyperactivity, restlessness, or an inability to sit still and relax — even in calm environments
  • Impulsive decisions, interrupting others, or acting before thinking with recurring negative consequences
  • Academic or professional underperformance despite intelligence, effort, and genuine desire to do better
  • A prior diagnosis that doesn't feel right — or no diagnosis at all despite years of struggling
The Assessment Process

What to Expect Step by Step

Our ADHD assessment is structured, transparent, and designed to give you clear answers — not more confusion. Here is exactly what happens from start to finish.

01
Request

Submit Your Intake & Get Scheduled Within 24 Hours

Complete our online intake form with your basic information and primary concerns. Our clinical coordinator will contact you within 24 hours to schedule your assessment and walk you through what to prepare — including any school records, previous evaluations, or medication history.

02
Interview

Structured Clinical Interview (Patient & Caregivers)

David Fowope, PMHNP-BC conducts an in-depth clinical interview covering developmental history, symptom onset and duration, home and school or work functioning, and any previous treatments or diagnoses. For children, caregiver input is essential and formally incorporated into the assessment.

03
Assess

Standardized Rating Scales & Behavioral Analysis

We administer validated, age-appropriate ADHD rating tools — such as the Conners, Vanderbilt, or Adult ADHD Self-Report Scale (ASRS) — to quantify symptom severity across multiple settings. We also carefully rule out co-occurring conditions like anxiety, depression, learning disabilities, or sleep disorders that frequently present alongside ADHD.

04
Diagnose

DSM-5 Diagnostic Formulation & Written Report

Using all gathered information, we develop a comprehensive DSM-5 diagnostic formulation. You receive a clear diagnosis (or ruling out of ADHD), a written summary of findings, and a plain-language explanation of what your results mean for your daily life — no clinical jargon, no confusion.

05
Plan

Personalized Treatment Plan, Letters & Next Steps

Every assessment concludes with a complete, individualized treatment plan — including medication recommendations when appropriate, behavioral and lifestyle strategies, therapy referrals, official accommodation letters for school or work (IEP, 504, ADA), and scheduled follow-up care to monitor progress.

Common Questions

Frequently Asked Questions

What is the minimum age for an ADHD assessment at Flourishington?
We conduct ADHD assessments for children ages 8 and older, adolescents, and adults of all ages. We do not have an upper age limit — adults who have never been evaluated and are seeking answers for a lifetime of struggles are very welcome. If you have a child younger than 8 who you are concerned about, please contact us to discuss what evaluation options may be available.
How is an ADHD assessment different from a regular psychiatric evaluation?
While a psychiatric evaluation is broad and covers all areas of mental health, an ADHD assessment is specifically focused on attention, executive function, hyperactivity, and impulsivity. It uses ADHD-specific standardized rating tools (like the Conners or Vanderbilt scales) and gathers information from multiple sources — including caregivers and teachers for children. An ADHD assessment also specifically produces documentation suitable for school or workplace accommodation requests.
Will my child receive medication as part of the assessment?
No — medication is never automatically prescribed as part of the assessment. If a diagnosis of ADHD is confirmed, we discuss all treatment options with you, which may include behavioral strategies, school accommodations, therapy referrals, and medication. Medication is always recommended only when clinically appropriate and with your full informed consent. We never pressure families or patients to choose any particular treatment path.
Can I get an accommodation letter for school or work from this assessment?
Yes. When an ADHD diagnosis is confirmed, we provide official clinical documentation suitable for IEP (Individualized Education Program) applications, 504 plan requests, college disability services, and ADA workplace accommodation requests. These letters are written on official clinical letterhead and include all required diagnostic and clinical information. If you need a specific format for your school or employer, let us know and we will accommodate.
What if I already have an ADHD diagnosis but want a second opinion?
Second opinions are entirely welcome and encouraged when you have doubts about a previous diagnosis. Many adults come to us after feeling that their diagnosis was rushed, that their treatment is not working, or that something was missed. We approach every evaluation with fresh eyes and do not assume previous diagnoses are correct. We will review any prior records you share with us and conduct our own independent, thorough assessment.
Does ADHD look different in girls and women?
Yes — significantly. ADHD in girls and women frequently presents as the inattentive type, which is less obvious and more easily missed than the hyperactive presentations more commonly seen in boys. Girls with ADHD often develop strong coping mechanisms that mask their difficulties, leading to late or missed diagnoses. Common signs include chronic daydreaming, emotional sensitivity, disorganization, difficulty completing tasks, and anxiety — often misdiagnosed as pure anxiety or mood disorders. At Flourishington, we are highly aware of these gender differences and actively assess for them.
