That is precisely what Care Coordination at Flourishington delivers.

David Fowope, PMHNP-BC takes a proactive approach to coordination, regularly communicating with your broader care team — with your written consent — to share relevant clinical information, align treatment strategies, and prevent the fragmented, disconnected care that too often leads to poorer outcomes. We do not simply prescribe and release. We stay connected.

For children and adolescents, this means working directly with school counselors, IEP teams, and teachers to ensure academic supports align with clinical recommendations. For adults, it means coordinating with employers, employee assistance programs, and primary care providers. For older adults, it means working alongside geriatric specialists, home health agencies, and family caregivers.

Before contacting any member of your care team, we obtain your written authorization specifying exactly who we may communicate with, what information may be shared, and for what purpose. Your privacy is always paramount — nothing is shared without your explicit consent.

Frequently Asked Questions

Is Care Coordination a separate service I need to request?
Care Coordination is integrated into all clinical services at Flourishington — it is part of how we practice, not an add-on. However, the intensity and scope of coordination varies. For patients with complex care needs, multiple providers, or school or workplace accommodation requirements, we actively discuss and formalize a coordination plan. For others, coordination may be lighter and more as-needed. Simply let us know during your intake what providers are involved in your care and we will take it from there.
Will my personal information be shared without my knowledge?
Absolutely not. All care coordination communication requires your explicit written authorization before any information is shared with any provider, school, employer, or family member. You specify exactly who may be contacted, what information may be shared, and for how long the authorization is valid. You may revoke authorization at any time. Full HIPAA compliance is maintained throughout every coordination activity.
What if I don't have a therapist or other providers yet?
No existing care team is needed to benefit from Care Coordination. If you do not yet have a therapist, primary care physician, or other providers, we can help you build one. We maintain a trusted referral network of licensed therapists, psychologists, social workers, and medical specialists and will make warm, personalized referrals based on your specific needs, location, and preferences — and follow up to ensure you have successfully connected.
Can you coordinate directly with my child's school?
Yes — school coordination is one of the most frequently requested aspects of our Care Coordination service. With your written authorization, we communicate directly with school counselors, special education coordinators, IEP team members, and teachers. We provide clinical documentation, explain diagnostic findings in accessible language for educators, and advocate for appropriate academic accommodations and supports. For IEP and 504 plan meetings, we can provide written summaries or participate in team discussions when needed.
Does Care Coordination cost extra?
Basic care coordination activities — such as sending clinical summaries to your existing providers or school — are included as part of your clinical care at Flourishington. More extensive coordination activities, such as attending IEP meetings, completing detailed written reports for schools or employers, or managing complex multi-provider communication over extended periods, may be billed separately. We will always discuss any additional fees with you transparently before proceeding with extended coordination activities.
Can you coordinate with providers in other states or countries?
Yes — care coordination is not limited by geography. With your written authorization, we can communicate with providers anywhere via secure messaging, phone, fax, or written correspondence. For patients who have recently relocated, who travel frequently, or who maintain care relationships with providers in their home state or country, we actively work to maintain clinical continuity regardless of physical distance. Please let us know about any out-of-area providers during your intake.
